New Health Resource for Seniors & Families
By Jack I. Retired R.N.

Over the years as a registered nurse, I saw how difficult it could be for seniors and families to keep medications, appointments, and health information organized – especially during busy or stressful times.
To help make this easier. I created a printable large-print guide:
“Seniors Medication & Health Tracker”
This simple PDF workbook is designed to help keep important information together in one place for:
- Daily medication tracking
- Blood pressure monitoring
- Doctor appointments
- Emergency information
- Personal health notes
The goal is not to replace medical care, but to help people stay more organized and prepared when discussing their health with their healthcare provider.
I tried to keep the format clear, unclutter, and senior-friendly
This guide is a now available Payhip https://payhip.com/SeniorMedicationHealthTrackerLargePrintPDF
Thank you to everyone who has encourage this project and shared ideas along the way.
“This information is for general education-alway check with your healthcare provider for personal medical advice.”
